Guardian Care Management SMS Terms

Overview

The Guardian Care Management Program provides patient communication via SMS. By opting in, you may receive text messages from your healthcare provider or care team related to:

• Care Coordination
• Appointment reminders and updates
• One‑time passcodes
• Secure links to access information shared by your provider

Message frequency may vary based on your care activity and communication with your provider.

Opt-Out Instructions

You may cancel SMS communications at any time. Text STOP to unsubscribe.

After sending STOP, you will receive a confirmation message indicating that you have been unsubscribed. You will no longer receive SMS messages from the Guardian Care Management Program.

If you wish to resume receiving messages, you may opt in again through your provider or care team.
